Try lowering the headrest a notch and it might be a bit better.

Too close to the neck isn't a safety issue but it is a comfort issue. Kids are more likely to play with the belt if it's close to the neck.

I thought I did that? But I will check again. I've looked at too many lately...

Just looking at the picture, it looks like you can move the headset down an inch or 2 and still have the guides be above his shoulder. Play around with the height, as it will move that shoulder belt around in his shoulder a bit as it's raised and lowered.
