You know, it sounds to me that they flipped him ff at 1 because that is still what people think you do, then realized the issue of the head slump and that is why nurse was holding it and why they looked into the special needs seat. If the nurse is always with them, then it doesn't seem to me that rear facing will be an issue.

my DS has hydrocephalus, he is shunted, he has tone issues (he does not have a trach, or some of the other issues mentioned so I can't speak to that). He did so much better in a rf seat at a 45 degree angle. I think a well reclined myride would be awesome for this kid. They're pretty affordable, last a long time, are comfortable, and they will be able to have it installed by a CPST. If the seat is properly reclined there shouldn't be head slump, but if high tone is an issue this child may not be able to have control over it. There was a while where DS's upper body "pulled" to one side (kwim?) and we couldn't do anything about it until it was worked out through PT, but being reclined certainly helped to keep his airways open
