This was in a 97 Mazda Protege...
I got to my destination, and as I was opening the door, happened to notice that the lap portion of the belt was jammed under the recline lever for the drivers' seat. The lever is outboard, way at the back of the seat, right beside the seatbelt anchor. So the belt was in the right place, and I had pulled it tight across my lap as usual. Except it was only tight to where it was hung up on the lever. There was a good 4" of slack between the lever & the anchor. I've been driving this car for 15 years and never anticipated or checked for that problem!
Anyways, something to be aware of if your seat has a lever next to the belt anchor.
