I recently worked with a mom of 1y/o twins both in spicca casts, one with legs casted wide, the other with legs casted straight ahead. Both had been riding in MA's, but neither was comfy (understatement), and both seats were padded with a pillow behind their backs (per the doctor ) in hopes of forcing them forward enough that their legs could hang off the sides.
I got them into Radian XT's which were perfect, exactly what they needed, and allowed them to have their legs hanging over the edges.
In terms of development these babies were a little delayed (obvious reasons, they're casted and can't move well) and so we went with the XT for additional head support. Basically the XT allowed their legs to be where they needed to be, and the wings and padding on the rest of the seat kept their bodies aligned as much as possible. It was just a really.good.fit all the way around.
