The U.S. Consumer Product Safety Commission (CPSC) warns consumers that some “metoo” clip-on table top chairs, imported by phil&teds USA Inc., of Fort Collins, Colo. put young children at risk of serious injury due to multiple safety hazards. CPSC is urging consumers to stop using some metoo clip-on chairs immediately. Tens of thousands of the affected metoo chairs may have been distributed since May 2006 for about $50 through philandteds.com, Amazon.com, Buy Buy Baby, Target, Toys R Us, other online retailers and a variety of independent juvenile specialty stores.

Six L's of Immokalee, Fla. is voluntarily recalling a single lot of grape tomatoes, because they have the potential to be contaminated with Salmonella. The specific lot was packed on April 11 and was comprised of grape tomatoes that can be identified by Cherry Berry lot code DW-H in either in clam shells or 20 lbs. cardboard containers. The product was distributed to North Carolina, South Carolina, Alabama, Florida, New Jersey, New York, Michigan, Pennsylvania, California, Georgia and Canada, and reached consumers through retail stores and restaurant distribution.

Taylor Farms Pacific, Inc. of Tracy, CA has been notified by grower Six L’s that a specific lot of grape tomatoes supplied to Taylor Farms Pacific may be contaminated with Salmonella. This lot of grape tomatoes was used in salad products made by Taylor Farms Pacific for Albertsons, Raley's, Safeway, Savemart, Sam's Club, & Walmart and is being voluntarily recalled as a precautionary measure.

Safeway is expanding its voluntary recall to include fresh kabobs made with grape tomatoes sold in our full-service meat counter in several states. The kabobs were made using grape tomatoes supplied by Taylor Farms and sourced from grower Six L's that were recalled due to possible Salmonella contamination. The recalled kabobs were sold between April 26, 2011 and May 3, 2011 and have a Sell By Date up to and including May 3, 2011. The kabobs were sold in Safeway, Vons, Pavilions and Pak N Save stores in Arizona, California, Colorado, Wyoming, Nebraska, Nevada, New Mexico, Oregon, South Dakota and Southwest Washington State.

Quinault Tribal Enterprise of Taholah, WA is recalling all canned seafood products including all Salmons, smoked and non-smoked; Tuna, smoked and non-smoked; Smoked Sturgeon; Minced Razor Clams; smoked Razor Clams; and smoked Steelhead. The canned seafood products are being recalled because they were not adequately processed according to the U.S. Food and Drug Administration's low-acid canned food regulations. The recalled products were distributed nationwide through distributors and retail stores.

Defibtech, LLC is initiating a worldwide voluntary recall of certain DDU-100 series semi-automatic external defibrillators (AEDs) sold under the Lifeline AED and ReviveR AED brand names, including 65,885 AEDs distributed in the United States. This corrective action addresses two possible conditions, which in rare cases may cause an affected AED to cancel shock during the charging process and not provide therapy which may result in failure to resuscitate the patient.

Bristol-Myers Squibb initiated a voluntary recall of one lot of 1,000-count bottles of Coumadin® (warfarin sodium) Crystalline 5 mg tablets. Bottles of 1,000 tablets are distributed to pharmacies for further dispensing to patients in prescription quantities. The lot number affected in the U.S. is 9H49374A with an expiry date of September 30, 2012. The recall is a precautionary measure based on the company’s testing of tablets from a returned bottle. A single tablet was found to be higher in potency than expected.
