Marathon harness tightening belt malfunction

A few weeks ago I looked in my rearview mirror to see my 3 year old dd leaning completely forward in her marathon....chest on her lap. I nearly had a heart attack, pulled over promptly and tightened the strap again, checking it at least a dozen times, LOL. I needed to wash the cover anyway, so took the Marathon out when I got home, checked everything over, checked for recalls...etc. There was a coating of fine crumbs on the adjuster strap (the one you pull from the center of the seat to tighten the harness). I wiped it off with a damp cloth, and tried to get it to slip again and could not.

I'm not sure exactly what happened, but I've tried lots of times to get it to do this again, and cannot. But I'm a little more than scared to put the seat back into use! She's been riding in the Nautilus we have for dh's car, but now he's going to need that, so what should I do?

Has anyone else had this problem? I know there was a recall for this type of problem, but our seat was not affected.

no, she wouldn't have been able to reach it herself without the harness being really loose. Before I cleaned it off, it was very slick and would slip--you had to actually push the little flip/lock back down to get it to stay. I don't know if there was a little fabric from the cover stuck in it or just the crumb residue.
