Solowave Recalls Tornado brand Home Playground Tube Slides with Port Holes Due to Laceration Hazard. Sold at. Toys R Us and other stores nationwide, and online at toysrus.com from February 2008 through August 2013 for between $1,300 and $1,800 as part of a play system, or between $275 and $370 for the slide only.

Trek Recalls Madone Bicycles Due to Crash Hazard; Front Brake Can Fail. Sold at Bicycle stores nationwide from July 2012 through December 2013 for between $3,400 and $6,300, and for custom models between $4,000 and $15,000.

Burley Design Recalls Tailwind Racks for Trailercycles Due to Fall Hazard. Sold at Independent bicycle retailers and specialty outdoor retailers nationwide and online at Amazon.com, Biketrailershop.com and other websites from November 2011 through September 2013. Individual racks sold for $65 and Kazoo and Piccolo trailercycles with Tailwind Racks sold for $300 and $350 respectively.

SRAM Recalls Bicycle chain Derailleurs for Bicycles Due to Fall Hazard. Sold at Specialty bicycle retailers nationwide from September 2012 to November 2013 for approximately $380 as an aftermarket installation. They were also sold as original equipment on new high-end bicycles.

PackTite Recalls Bed Bug Heating Units Due to Fire Hazard. Sold at Pest control companies and pest control product distributors nationwide, including Atlantic Pest Solutions, Bedbug Central, Bedbug Supply, Broadway Exterminating, Dana Pest Control, Do My Own Pest Control, Grainger, M&M Environmental Isotech, K-9 Sweeps, Orkin, Pestaway, Round-the-Clock Pest Control, Steritech, Stern Environmental, Tallman Scientific, Univar and US Bedbugs from October 2009 through January 2013 for between $300 and $330 for the system and $53 for the heater when sold separately.

efco brand Gas Trimmers from Emak USA Recalled by efco Due to Fire Hazard. Sold at Authorized efco dealers at both retail stores and online, and Menards stores between June 2009 and July 2013 for about $400.

Eight Retailers Recall 32” Coby Flat Screen Televisions Due to Fire and Burn Hazards. Sold at ABC Warehouse, Fry’s Electronics, h.h. gregg, Nebraska Furniture Mart, P.C. Richard & Son, Sears/Kmart, Toys R Us and online at BestBuy.com, dealtree.com, techliquidators.com and bestbuy.dtdeals.com, and other stores and sites nationwide, from August 2011 through November 2013 for between $170 and $260.

Shaw Industries Recalls Aristocrat II Carpet Due to Fire Hazard. Sold exclusively at Lowe’s stores nationwide between June 2013 and September 2013 for about $1 a square foot.

Michelin North America, Inc., (Michelin) is recalling certain Michelin LTX M/S tires, size LT225/75R16 115/112R LRE, manufactured from January 10, 2010, through June 23, 2012, and supplied for use as original equipment on certain Ford vehicles, and as replacement equipment on various brand commercial light trucks, full-sized heavy duty vans, small RVs and some large pickup trucks. These tires could experience tread separation.

Bridgestone Americas Tire Operations LLC (Bridgestone) is recalling certain H300 315/80R22.5 18 ply tires manufactured January 2012 through August 2013 and sold to customers in Guam. The affected tires may lose pressure from a crack at the inner liner splice. Such a crack could penetrate into the sidewall, resulting in a bulge on the sidewall.

Zip International Group LLC, is recalling herring fillet in oil 400 gram in plastic packaging due to Listeria contamination. The recalled product was sold to retail grocery stores in New York State beginning on 11/01/2013 and ending on 11/14/2013.

Cargill's animal nutrition business today announced a voluntary recall of two of its Nutrena NatureWise poultry feeds due to incorrect levels of calcium. The affected products were manufactured at Cargill's facilities in Flora, Ill., Oklahoma City, Okla., Abilene, Texas, and Mineola, Texas, between May 1, 2013 and Nov. 21, 2013.

Flat Creek Farm & Dairy of Swainsboro, GA has recalled 200 pounds of Heavenly Blue cheese, because of potential contamination. Heavenly Blue was distributed in certain parts of Alabama, Georgia and Florida via and (3) online orders (www.flatcreekdairy.com)

Yauk’s Specialty Meats, a Windsor, Colo., establishment, is expanding its recall to include an undetermined amount of various meat and poultry products that were produced under insanitary conditions. Brands include: Four Sisters Farm, Heart Rock Bison, High Point Bison, Luc’s Pizza, Mountain States Poultry & Meats, Open A Bar 2, Rocky Plains Quality Meats, Schmidt’s Bakery & Deli, Wag’s Livestock, Wayne’s Specialty Meats, Windsor Dairy, Wyoming Pure Beef, Yauk’s Specialty Meats, Colorado’s Best Beef, James Ranch, Rocky Plains Meats, Long Family Farms, Horned Beef, Mile High Hungarian Sausage. The products were sold at the wholesale and retail level in Colorado, Nebraska, New Mexico, Utah and Wyoming.

Santa Maria Foods, a Brampton, Ontario, establishment, is recalling approximately 2,600 pounds of whole boneless ham prosciutto product due to possible contamination with Listeria monocytogenes. The ham product was shipped to California and Michigan for further distribution.

Mary Ann’s Specialty Foods, a Webster City, Iowa, establishment, is recalling approximately 3,536 pounds of Niman Ranch Applewood Smoked Uncured Petite Ham because the establishment number was not included on the label. The products were distributed to retail stores nationwide. These products were produced between Oct. 23, 2013 and Oct. 31, 2013.

CLR Medicals International Inc., initiated a nationwide recall of Viscocel and Viscocel Plus, in response to FDA inspectional findings of deficiencies in the Quality System regulations for Medical Device Manufacturers. This recall includes all lots of Viscocel and Viscocel Plus, manufactured from September 01, 2011 to September 19, 2013.

Abbott today announced it is initiating a voluntary recall of 20 lots of FreeStyle® and FreeStyle Lite® Blood Glucose Test Strips in the United States. These lots of test strips may produce erroneously low blood glucose results when used with both "FreeStyle® Blood Glucose Meter" and "FreeStyle® Flash Blood Glucose Meter” [neither of which have been in production since 2010], as well as the OmniPod® Insulin Management System.

Baxter International Inc. announced today it has initiated a voluntary recall of one lot of Nitroglycerin in 5% Dextrose Injection due to particulate matter found in one vial. IQ Formulations, of Sunrise, Florida is initiating a precautionary and proactive recall of all lots of its 45-capsule bottles of HYDRAVAX due to potential inclusion of an unlisted ingredient. The recalled HYDRAVAX was distributed nationwide in retail stores and mail order.
