Here are some common mis use and a crash test showing what happens with each. The dummy in the foreground is in properly.

Loose install:

Are these videos? I can only see pictures.

How do you share on FB? I'd never heard of that. Can you do it via the app.

Where are these videos from originally? And does anyone happen to know if the top tether is connected on the first few with the forward-facers? I can't tell in the video.

Yes, they are videos. I don't use the app, so don't know.

I got them from one of my trainings. I believe I first posted these in 2010. This is the second thread.
