The lady walked away and the driver of the truck had minor injuries... well, luckily the ADULTS HAD SEATBELTS!!! WTH!?!?!?!?!
I teared up when I realized that there's a blue blanket draped out the rear window with what looks like footballs... DSS2 ALWAYS rides with his blanket, he as a specific 'car' blanket.

This is terrible! This is also why I am SO glad that I know how to properly restrain my child! I hope I never have to test my seats, but I am glad that I have my son in them just in case!

Maybe an overhead shield seat without the straps done properly. I remember 13 years ago I went with my cousin to pick up his son from somewhere. The baby was around 13 months at the time and I watched mom put him in his FF OHS seat and told him "arms up" so that his whole upper body went through the harness restraint and the shoulder straps were around his waist once she clicked the buckle and pulled the shield down. I wonder if that was a possibility for this poor little guy. So sad to know that he might have been okay if he had been appropriately restrained. :(

This is terrible, so sad.

It said they were looking into how the seat was restrained as well as the child. Hmm.

it could have happened for any number of reasons: OHS, loose straps, straps missing, expired seat, defective seat (britax with a frayed adjuster strap? recaro with a defective adjuster?)... but wild speculation doesn't really help.

“The car seat placement and means of securement of the seat and child are under investigation,” Officer Brad Baehr said.

Terrible. We just had a similar one in Wisconsin. The six-year-old in a seatbelt survived, but the 22-month-old in a car seat was ejected, still in the seat. All so sad and preventable.

...Investigators said passenger Ishaneeka I. Marsh, 27, of Milwaukee, died at the scene of the crash, which occurred on Interstate 43 between Cedar Grove and Oostburg. Her 22-month-old son, Iahm D. Marsh, died five hours after the crash at Children’s Hospital of Wisconsin in Wauwatosa.

...Iman Marsh, 6, who was seated behind Kelly and wearing a seatbelt, was hospitalized but has since been released, officials said. Iahm was belted into a child safety seat, but the seat was thrown from the car during the crash with the boy still inside. The two adults were not wearing seatbelts.

OverHead Shield

Some parents think they're safer (they're not: "shield slap" anyone?)

They also tended to have a difficult buckle that might not always (easily/obviously) be securely buckled. There was a presentation, in part, about this at my region NHTSA conference a couple years ago by one of the guys who does the car seat crash testing. They had a video showing the crash simulation of a properly buckled OHS vs. an improperly buckled OHS -- the improperly buckled OHS dummy submarined out, being partially decapitated in the process....
