Techs - which form do you use for private checks?

I just completed my tech class. We used the Safe Kids form at our seat check (did not like it at all), and there are several other forms in the manual. Which form do you use when you do checks on your own? I want one that has a good liability waiver that parents have to sign and is easy to use. It seems that I'd have to modify an existing form if I used one from the manual (since they mention specific police departments, counties, etc.). Any suggestions would be great. If you have one that you can email me, even better. I can pm my email address.

P.S. I have tried to join the tech forum with no luck. If anyone in charge reads this, please help!

I use the SK one as well as 2 from the training manual that I modified. I did use the disclaimer from one and put it on the other. COngrats on finishing your class. Definately document everyhing know matter howteidious it may seem. This was drilled into our heads in class. I made my own sister sign one when I helped her.

It took about 2 days for someone to get me into the tech forum and be able to post. You could PM a mod or Admin.

Thanks. I failed to follow this part of the instructions properly and now it won't let me fix it:

"Be sure to include your full name, address, zip code and technician or instructor number where it asks for a "Request Reason".

I also noticed that my info was not public at Safekids, but it is now. I gave my tech ID number previously, so perhaps making my info public with Safekids will be enough.

That was the problem. I've been waiting for the SK page to be updated. I noticed earlier that you said you finished the class on Monday and some instructors take several days to input the information, so I was waiting until next week before PMing you about it. You're in now :).

Oh, and to answer your original question, I use the dumb SK form because my SK coalition counts what I do during private checks toward their yearly totals. I also send any donation I receive to them as well. Dumbest form I've ever seen . . .
