Big kid with bad belt fit?

My lovely nephew (who is nearly as car seat crazy as I am, and often helps me out when my DH is gone) is 15 years old, 5'8", and about 130 pounds. He's far past the stage of boosters, but he often winds up with bad belt fit in many vehicles. It's not nearly as horrendous as on a small child, but he often ends up with gaps between the shoulder belt and his collar bone or the shoulder belt is on his neck or the edge of his shoulder. Is there anything we can do to fix this? We've tried adjusting the shoulder belt height when available but if often doesn't help.

If he's sitting in the front seat or driving, he can move the seat forward a notch or recline the seat back less. I am smaller than your nephew and I find when I get a gap, it's typically because I don't have the seat adjusted properly for me.

He has quite long legs (much like the rest of our family) and we don't want his legs too close to the dash if we can help it. We've been able to adjust the seat to give him good fit in his mom's van, but otherwise he's either crunched or the belt fit still isn't good.
