Question clek oobr belt shoulder guide

The belt is always falling out of the guide. I put it fully in and it still comes out 2 or more times per week. Clek says the belt should be removed from it upon exit from the seat and reinserted each new ride. I don't know when it comes out, if it's on the child's exit from the seat or its a particular movement while buckled (not that the child is all over the place). Guide is in proper position just above the shoulder.
Is this common?
It's outboard 3rd row in a 2013 sienna.

In my experience when this happens it's more about the belt than the booster. Belts that do that tend to do so with most boosters. There are only a few belt guides that are shaped in a way to prevent this in most vehicles with the issue. You may want to try a different seating position where the belt hits differently.
