Well, getting a car seat with a shape that works well for the child and fits well, making sure it's properly reclined. Rolled blankets can be added at the sides for support if needed.

I don't know what you mean by having a shape that fits the baby...aren't they all essentially L shape? What's something shaped different than say, a Keyfit, that might be suitable here?

Usually ditching any non-mandatory padding (like the KeyFit headrest, it's very head-pushy-forwardy for a lot of babies, or the fairly useless head rainbow things in the Gracos) and really taking care to get full recline can make kids stay better in position, and like KQ said, rolled receiving blankets on the side of the head help. Also making sure the crotch strap is as far in as it can go help keep wee ones from slouching.
(Didn't mean to echo KQ totally, just had some other thoughts to add for any lurkers :))

Actually most rear facing only seats, like the Keyfit, are very C shaped. The Safety 1st OnBoard would be an example of a more straight backed L shaped seat.

Just on this note, I had success using this technique and using "ouchless" hair elastics (without the metal piece) to keep the blankets rolled. I probably wouldn't' use them with an older child, but for an infant it is sure handy to not have them unfurl all the time!
