2 1/2 year old with kyphosis

We have a 2 1/2 year old with congenital kyphosis and scoliosis. He complains that "my back is hurting" when he rides in a car seat for more than an hour. He is 29 pounds, forward facing in a Graco Smart Seat. What can we do to make him more comfortable, but still safe? Grandma suggested a small donut shaped pillow to protect his curved spine from being pushed against the seat?

Welcome!

Rear-facing might remove some pressure my changing what is weight-bearing. It's not safe to put a donut pillow behind. Nothing that isn't approved with the seat should be added between the body and harness. Which part of the back is hurting?

I can only imagine the hurt is on the outward poking curve part of his spine which is about 2/3 of the way up his back. Guess I've never really asked the exactly where does it hurt question.
I never thought about the different angle on rear facing. We could turn him back to rear face for a longer trip and see if the complaints stop.
