I need a form! :)

Our goal as police officers, believe it or not, is education and voluntary compliance.

In a show of good faith in that goal I'd like a form I can give out whenever I cite someone for child restraint violation (for the record, it's looked down upon to cite for "minor" misuse--ie, if I notice LATCH and seatbelt, or if straps below shoulders FF, that kind of stuff--they're looking for unbuckled, non restrained, or tiny infant FF, etc----and believe me I see enough of those that I actually get excited when I see "minor" misuse!).

I need it to look spectacularly professional... have a notice that they have X number of days from X date to see a CPST, and have a spot at the bottom for a tech's signature, address, phone number, tech number, or whatever identifying info y'all use, and that if the conditions are met by said date, their citation would be reduced to a warning. And some kind of "upon completion of this form please fax or bring in person to (address/fax/etc).", a space for their CRN number and citation number and officer name and badge number. And a comments section for the tech.

I know that's completely disorganized and a bunch of info. Can anyone help me? Does anyone already have a form like this that can maybe be slightly modified?

I'll be calling techs in our county (1!) and the surrounding counties in the next few days to see which ones would be willing to participate. I know one in Leesburg that will for sure (which isn't far for half the people I pull over---we are kind of between two military bases so I'll need to cover a 4 county area in finding techs willing to participate).

THANK YOU FOR YOUR HELP!

Such a form doesn't have to be created, vetted, and legally passed by your jurisdiction?

I'll have to present it to my assistant chief once I have it. As it is, there's a window of about 10 days where tickets that WE issue can be reduced to warnings at OUR discretion. Most commonly this is in the form of an expired registration (if it is recently expired and you can show that you renewed it within 10 days it'll be changed to a warning... Not all officers do it, but I know several do).

I'm pretty good at that sort of thing, and if I can come up with the time I could probably create such a form... whether I can come up with the time within the next week or two is another issue entirely. I have a lot going on right now. But if something else doesn't pop up before I find myself with some free time on my hands, I'll see what I can come up with.

As an aside... you might consider carrying the USAA booklets (they're free!!) to pass out to people you happen to be pulling over when you also happen to see minor mis-use... along with an encouragement to meet with a CPST even when the issue isn't severe enough to warrant a citation.

You might contact Stephanie at SafetyBeltSafe- SBS ran the violator program in CA for many years (that allowed people to get a ticket wiped from their record if they completed an education class, similar to traffic school for a speeding ticket.)

I just ordered 30 more. All mine are already gone

Hi,
I just joined so I could respond to this post, a coworker forwarded it to me. I'm from NC and we have a CPS Diversion program we run in about half of our counties now (and continually adding more) It's basically where police write tickets and in order to get CPS ticket waived the parent/caregiver has to get their seat checked. There is more to it than that, including the type of forms you are referring to and DAs support. Please send an email to me at renae.harris@ncdoi.gov and I'd be glad to get you copies and more info on our program. Thank!
