The instructions I have say that after you switch the SIM out for one on your preferred carrier (one that is compatible with SIM) and set up as new iPhone or restore from your own backup, the phone should work on your carrier. iTunes gave me a "Congratulations, your iPhone has been unlocked." message.

I received this phone from Apple in August so it's relatively new. It was a warranty replacement so it was probably an Apple Refirb which is pretty much brand new. I don't keep a screen protector on it so it has some minor scratches on the bezel but you have to look close. There aren't any nicks to the surface or cracks. Everything works fine.
It comes with nothing. Seriously, you don't get headphones, wall charger, USB, or the original box. I will ship it with love and insurance.
