It depends on whether you're measuring for a rear-facing or forward-facing seat. Rear-facing you need the head contained within the shell with at least 1" to spare (usually) so you'll want to measure to the top of the head. Forward-facing, the straps need to be at or above the shoulders, so you'd measure to the shoulders. (The head also needs support up to the top of the ears, but as long as the shoulders still fit in the harness, the head is usually fine, so you don't need to worry about that.)

That said, seated height limits are a good ballpark, but measuring seat and strap heights isn't an exact science, and kids can lose/gain room when seats are installed. So use it for a rough idea, but don't put too much stock in the measurements.

Okay thank you. It's for the recaro we have and it was brought up that dd was getting close to the 22.5" limit.
Also generally how long does it take for a child to gain 1" in seated height?
Dd is 19" from bottom to top of head so trying to estimate of how much longer it will last.

It really depends on the kid and the age. A 6-month-old will gain an inch a lot quicker than a 3-year-old (although even they can have growth spurts). How old is she?
