Pottery Barn Kids Recalls Sweet Lambie Crib Bumpers Due to Entanglement Hazard. Sold at Pottery Barn Kids stores and catalogs and Pottery Barn Outlet stores nationwide, and online at potterybarnkids.com from April 2009 through July 2012 for between $129 and $149.

Fred Meyer Recalls “Chicken Dance” Easter Chicks Due to Hearing Damage Hazard. Sold at Select Fred Meyer stores in Oregon, Washington, Idaho and Alaska from February to March 2013 for about $20.

H&M Reannounces Recall of Children’s Water Bottles Due to Choking Hazard. Sold exclusively at H&M stores with children's departments nationwide from July 2012 through March 2013 for between $1.50 and $5. About 200 of the bottles were sold after the original recall was announced in September 2012.

Vive La Fete Recalls Children’s Pajamas Due to Violation of Federal Flammability Standard. Sold at Boutiques, children’s specialty stores nationwide and online at www.vivelafete.com from September 2012 to January 2013 for about $40.

Teavana Recalls Glass Tea Tumblers Due To Laceration and Burn Hazards. Sold exclusively at Teavana stores and online at Teavana.com from August 2007 through May 2013 for about $15 to $33 for the individual tumblers, for about $40 for the Flourish Iced Tea Glasses Sets, and for about $80 to $100 for the Imperial Blooming Collection Tea Sets.

GNU Snowboard Bindings Recalled by Mervin Manufacturing Due to Fall Hazard. Sold at Specialty Sports Inc., REI and other national, small chain and independent sporting goods stores, and online at www.backcountry.com and www.mervin.com from August 2012 to March 2013 for between $125 and $310.

KTM North America Recalls KTM and Husaberg Motorcycles Due to Crash Hazard. Sold at Authorized KTM and Husaberg dealerships nationwide from January 2012 to April 2013 for between $ 5,300 and $ 8,450.

Main Street Family Pharmacy, LLC today announced a voluntary nationwide recall of all lots of all sterile products compounded by the pharmacy. The compounded products that are subject to the recall are those products with a use by date on or before November 20, 2013. The recall is being initiated due to seven (7) reported cases of adverse events in the form of skin abscesses, one of which appears to be fungal in nature. These products were supplied to the offices of licensed medical professionals and patients.

Fresenius Kabi USA announced today it has initiated a voluntary recall of one lot – Lot 6103882 – of Magnesium Sulfate Injection, USP due to the potential presence of glass particles in the vials. The product was shipped in the United States between May 30, 2012 and June 6, 2012 and has an expiration date of October 31, 2014.

FRUIT TREASURE of CHULA VISTA, CA is recalling 43 boxes (25lb) of fresh “JAMMIN PEPPERS” THAI PEPPERS, because it has the potential to be contaminated with Salmonella. The recalled fresh Thai peppers were distributed on April 7, 2013 to April 10, 2013 to retail markets in California.

Lowlite Investments d/b/a Olympia Pharmacy ("Lowlite") today announced a voluntary multi-state recall of all sterile drug products compounded by the pharmacy that have not reached the expiration date listed on the product. The recall is being initiated due to concerns associated with prior quality control procedures that impacted sterility assurance.

Not an official recall, but will include it anyway:
Packages of frozen berries sold at Costco are believed to be linked to an outbreak of hepatitis A cases in Colorado, New Mexico, Nevada, Arizona and California. The suspected source of the outbreak is Townsend Farms Organic Antioxidant Blend frozen berries purchased from Costco stores. The blend includes cherries, blueberries, pomegranate seeds, raspberries and strawberries.
