Surly Bikes Recalls Bicycle Forks Due to Fall Hazard. Sold at Bicycle stores nationwide and on various websites from May 2012 through February 2013 for about $100 individually or on Surly Pugsley bicycles for about $1,750.

Family Dollar Stores Recalls Optimus Tower Quartz Heaters Due to Overheating and Fire Hazards. Sold exclusively at Family Dollar Stores from September 2012 through December 2012 for about $35.

Meijer Recalls Touch Point Baseboard Convection Heaters Due to a Fire Hazard. Sold exclusively at Meijer stores from September 2011 through February 2013 for about $50.

Toro Recalls Toro® Z Master® Riding Mowers Due to Fire Hazard. Sold at Toro dealers nationwide from January 2012 through April 2013 for between $7,700 and $8,700.

Kumho Tire U.S.A., Inc. (Kumho) is recalling certain Kumho Solus KH25 passenger car tires, size 225/45R17, produced in the weeks of 3411 through 2512 (August 21, 2011 through June 23, 2012). The tires resold by Liberty have 3 drill hole punctures through the tread surface that if repaired, may not adhere to specific Rubber Manufacturers Association (RMA) standards for tire repairs, which could result in rapid air loss while the vehicle is in use and result in a tire failure that could cause a crash.

Weston, Florida: Apotex Corp. announced today that it is conducting, on behalf of the manufacturer Hospira, Inc., a voluntary nationwide recall of 21 lots of Piperacillin and Tazobactam for Injection, USP 40.5 grams, NDC number 60505-0773-00 to the hospital / healthcare provider /user level. The impacted lots of Piperacillin and Tazobactam for Injection, USP 40.5 grams may show precipitation / crystallization in IV bag or IV line after reconstitution.

Tibor’s Gourmet, a Palmdale, Calif., establishment, is recalling approximately 200 pounds of ready-to-eat smoked pork sausage products because they were produced without the benefit of federal inspection. The products were produced between Feb. 25, 2013, and May 2, 2013, and shipped to a retail chain in Los Angeles and Orange counties.

Krinos Foods, LLC. is Expanding its April 28, 2013 Recall to Include Additional Expiration Dates Jan 01-2014 to Jun 08-2014 of Tahini Products Because of Possible Health Risk. The Tahini product was distributed nationwide through retail stores.

Chang Kwung announced today that it is conducting a voluntary nationwide recall of the company’s dietary supplements sold under the brand name Lightning Rod 500 mg per capsule packaged in 3-count, UPC 6 89076 20257 2 and 12-count bottles, UPC 6 89076 20297 8. It is sold nationwide via internet in 3 capsule count and 12 capsule count bottles between August 2012 and May 3, 2013.

BeaMonstar Products is voluntarily recalling all of SexVoltz brand, Velextra brand, Amerect capsules to the consumer level. Laboratory analysis conducted by the FDA on SexVoltz and Velextra has determined these products contain undeclared tadalafil. SexVoltz, Velextra, and Amerect was distributed Nationwide to wholesalers, retail, and via internet.

Bimeda Inc., on behalf of the manufacturer, Bimeda-MTC Animal Health Inc. is voluntarily recalling one lot of Calcium Gluconate, one lot of Dextrose 50%, and one lot of Hypertonic Saline, all of which are in the form of injectable fluids, to the user level. These injectable fluids are being recalled as a precautionary measure; because of departures from customary sterility procedures, sterility cannot be entirely assured.

The Compounding Shop, LLC is conducting a voluntary recall of all lots of sterile products compounded by the pharmacy that are not expired. The recall is being initiated due to concerns associated with quality control procedures that present a potential risk to sterility assurance that were observed during a recent FDA inspection.

Cargill’s animal nutrition business today announced a voluntary recall of Showmaster® S-Series Show Lamb Feed (BW) because it contained incorrect sodium molybdate levels. The affected products were manufactured at Cargill’s Burlington, Washington facility between March 1, 2013 and March 15, 2013 and were sold only in Washington and Idaho.
