How does the belt otherwise fit in terms of his hips? Do his knees bend at the edge of the vehicle seat and do his feet sit flat on the floor?
What year and model of vehicle do you drive? Some vehicle manufacturers are putting comfort guides in to their vehicles now - they're typically a little plastic piece on an elastic and the shoulder belt just slides in to it. You can also check to see if the height of the shoulder belt can be adjusted - it is sometimes adjustable in the 2nd row of some vehicles.
