In the 2004-2010 Sienna manual it gives directions on installing ffing seats in the second row. Replacing the headrest is part of the directions. I emailed Toyota to find out if that headrest was required. Here's the response I was given:

We appreciate your interest in the Sienna.
Generally, we would not have additional information beyond what is in the manual. We do note that the manual does not offer any Caution statements about not replacing the headrest, and we would agree with you that if the child seat fits better without it, you may be better off without the headrest.
Do keep in mind, however, that if the headrest is removed, it would be best to remove it entirely from the vehicle, as it may become a projectile in a collision situation.
One other consideration is that the posts of the headrest may assist in keeping the tether strap centered on the seat. If you opt to go without the headrest, we would encourage you to closely monitor this, and ensure that the strap remain securely in place, not slipping to the side. Also consider that t may not be possible to fully presume or simulate the inertia generated in collision; though the strap may feel quite tight and secure, the forces in a collision my be unexpectedly great.
