Outgrowing forward facing

What is the rule with regard to ears? Do any seats say use until middle of ears even with shell?

A forward facing seat is outgrown when the weight limit is met, or the tops of the ears go over the top of the shell, or the shoulders go over the top slots. The only exception to that is the Radian, which allows use with shoulders over the top slots under certain circumstances, but still requires top of ears and weight limits.
